Seventh Grade Health Requirements
- None
New Students to Illinois:
- Parent or guardian must accompany students to registration to sign required forms.
- Child’s social security number is required
- Certified copy of birth certificate
- Completed physical on Illinois form
- Up-to-date immunization record
- Vision screening
