Sophomore Health Requirements

  • None

New Students to Illinois:

  • Parent or guardian must accompany students to registration to sign required forms.
  • Child’s social security number is required
  • Certified copy of birth certificate
  • Completed physical on Illinois form
  • Up-to-date immunization record
  • Vision screening

Sophomore Fees - $53 plus additional fees based on the individual classes they have signed-up to take.

  • $10.00 fee for *Calculus, *US Studies, and *Web Page Design. 
  • $15.00 fee for Guitar, Intro to Theatre Arts, Theatre Arts, and Drawing/Painting. 
  • $20.00 fee for 21st Century Foods, *Accounting 1, Accounting 2, *Desktop Publishing I and Shakespeare, Advanced Biology, Advanced Chemistry, BSAA, *Algebra III, Sculpture, STEM, Intro to Engineering & Tech, *Lit. and Comp. IV C.P., Orient. to Energy Technology, and Orient to Communication Technology, *Pre-Calculus, *Work Ethics. A $25.00 fee for Ag. Manufacturing, Ag. Mechanics, Ag. Construction, and Anatomy. 
  • $35.00 fee for Foods & Nutrition. A $40.00 fee for *Lit. & Comp. IV-AP. 
  • $75.00 fee for *Speech and *Psychology. 
  • $100.00 fee for Driver Education.
