Kindergarten:
Physical examination record
(licensed Physician, PA or NP/dated within 1 year of start of school)
Lead screening test record
Immunization record
(up to date per Department of Public Health requirements)
Dental examination
(that has been completed within the last 18 months)
Vision examination
2nd Grade:
Dental examination
(that has been completed within the last 18 months)
5th Grade:
Record of Hepatitis B immunization (series of 3)
6th Grade:
Physical examination record
(within 1 year of start of 6th Grade)
Dental examination
(that has been completed within the last 18 months)
9th Grade:
Physical examination record
(within 1 year of start of 9th Grade)
Record of Tetanus/Diphtheria or Tetanus/Diphtheria/Pertussis booster
Sports Participation:
Students must have a current physical examination record on file, stating that the student is able to participate.
(within 1 year of tryouts)
Transfer Students:
In State-copies of previous school’s health records
Out of State-copies of immunization records
New physical examination record
