Sports Physical
4th, 5th, 7th, 8th, 10th, 11th and 12th Grades

 

Physical activity is always encouraged for our students to promote a healthy lifestyle.  There are many interscholastic sports activities offered at Hamilton Co. Unit 10.

            Please remember that Hamilton Co. Unit 10 follows the Illinois Jr. High & Sr. High Athletic Association’s guidelines re. sports physical exam requirements for anyone wishing to participate in tryouts or practice for a specific sports activity.

            Those requirements state that no student shall be permitted to compete in any practice or game (including tryouts) unless the student has on file a current (not more than 12 months preceding such tryout, practice or game) sports physical exam that states the student is physically fit to participate.  The sports physical must be completed by a licensed Physician, PA or NP.

            The sports physical exams must be given to Health Services at the Jr./Sr. High School for filing and determination of sports eligibility.  If you feel that your child may want to participate in any sport during the school year, we advise that you get a physical completed in the summer prior to the start of that school year.  Remember, the physician will not automatically send the school the completed physical.  Either you or your student should bring the completed physical to school.

Current physicals are also required for students’ trying out for sports/cheerleading activities, (within the last 12 months of the participating sporting event).  Documentation must state that the student is able to participate in the activity.  The required 6th and 9th grade school physical examination will also work as the sports physical for that year (if it is within 12 months of the start of try-out practice).
