Kindergarten

Child entering kindergarten are required by law to have the following on file:

1.  Physical Examination (dated within one year of starting school)

2.  Dental Examination (within the last 18 months)

3.  Eye Examination

4.  Required Immunizations (Please note:  It is mandatory for the record to state the month, day and year of inoculation

5.  Lead Screening Date

6.  Copy of original birth certificate (not the hospital copy)

7.  Social Security Number

8.  Public aid number (if applicable)

 

The required immunizations include (following Illinois Department of Public Health guidelines):

1.  Diphtheria, Pertussis and Tetanus (DPT)

2.  Rubeola

3.  Rubella

4.  Polio

5.  Mumps

6.  Varicella  (on or after 1st birthday or proof of disease)

7.  Haemophilus influenza type B (HIB)

8.  Hepatitis (recommended but not required until 5th grade)

 

Immunizations and lead screenings may be obtained through the Hamilton County Health Department (643-3522) located in the Hamilton County Courthouse.  Please remember to take your child’s immunization record with you.

All records should be brought to your child’s school by the first day of attendance in the Fall.  In order for these requirements to be in place prior to the first day of school, parents should begin now.

Students will not be allowed to attend effective October 15th unless all requirements are complete or appointments are verified for completion.

 

2nd Grade

Child entering second are required by law to have the following on file:

1.  Dental examination (completed within the last 18 months)

 

5th Grade

The required immunization include (following Illinois Department of Public Health guidelines):

1.  Record of Hepatitis B immunization (series of 3)

Immunizations may be obtained through the Hamilton County Health Department (643-3522) located in the Hamilton County Courthouse.  Please remember to take your child’s immunization record with you.

All records should be brought to your child’s school by the first day of attendance in the Fall.  In order for these requirements to be in place prior to the first day of school, parents should begin now.

6th Grade

Child entering sixth grade are required by law to have the following on file:

1.  Physical Examination (within 1 year start of 6th grade)

Will also work as sports physical

2.  Dental Examination (completed within the last 18 months)

3. Tdap booster

4. 2nd Varicella

5. Meningococcal

9th Grade

Child entering ninth grade are required by law to have the following on file:

1.  Physical Examination (within 1 year of start of 9th grade)

Will also work as Sports physical

2.  Record of Tetanus/Diphtheria or Tetanus/Diphtheria/Pertussis booster

3. 2nd Varicella

12th Grade

Child entering twelfth grade are required by law to have the following on file: 

1. 2 dosage of Meningococcal (Only one dose required if first dose was received after 16th birthday)


Immunizations may be obtained through the Hamilton County Health Department (643-3522) located in the Hamilton County Courthouse.  Please remember to take your child’s immunization record with you.

They also can be obtained through the Hamilton Memorial Family clinic (643-2988) located in the Hamilton Memorial Hospital. Please remember to take your child's immunization with you. They have to have the immunization record for three days before giving immunizations.
